Understanding Your Options


The best route depends on the condition being treated, your health history, convenience, preferences, cost, and how easily a dose can be adjusted.

  • Transdermal therapy — patches, gels, or creams delivered through the skin
  • Oral therapy — some hormones and related medications as tablets or capsules
  • Injections — testosterone injections for some men with confirmed hypogonadism, with ongoing monitoring
  • Local vaginal therapy — low-dose local estrogen for vaginal or urinary symptoms of menopause
  • Pellet therapy — hormone released over time; because a pellet cannot be quickly removed or dose-adjusted, benefits and alternatives deserve careful discussion
  • Nonhormonal support — prescription options, sleep care, resistance training, nutrition, and management of related conditions

“Bioidentical” means the hormone has the same molecular structure as one made by the body. The term does not by itself mean a product is safer, more effective, or FDA approved. FDA-approved bioidentical options exist; many compounded preparations and pellets are not FDA approved.

Who May Need a Different Approach?


Hormone therapy may not be appropriate for everyone. Special caution, additional evaluation, or specialist collaboration may be needed with a history of certain hormone-sensitive cancers; unexplained vaginal bleeding; blood clots or clotting disorders; stroke, heart attack, or significant cardiovascular disease; severe untreated sleep apnea; elevated hematocrit; significant prostate concerns; active liver disease; pregnancy; or a desire for near-term fertility.

Testosterone can suppress sperm production and should not be started without a fertility discussion when future biological children are a goal. For women with a uterus who use systemic estrogen, endometrial protection generally must be addressed. Seek urgent care for chest pain, sudden shortness of breath, coughing blood, one-sided leg swelling, symptoms of stroke, or severe allergic reaction.
